Athletics Pre-Participation Information & Forms

ATHLETE CODE OF CONDUCT

- The KCS Athlete Code of Conduct MUST be reviewed, signed, and returned to the KCS Athletics Department prior to participation in ANY middle/high school sport or activity. Athletes in grades 9-12 also need to complete and sign the Random Drug Testing and Procedures consent form (pg. 8) and return that as well.
 
- Students in middle school must also review the middle school athletic grading policy listed below under the handbook.

Section 10. It is required that no student be permitted to participate in practice sessions or in athletic contests until there is on file with the principal a pre-participation medical evaluation form signed by a doctor of medicine, osteopathic physician, physician assistant, or certified nurse practitioner stating that the student has passed a physical examination, not prior to April 15, and that in their opinion the student is physically fit to participate in interscholastic athletics. In lieu of the form, the principal may accept a signed statement from the health care provider certifying that the student has passed a physical examination that encompasses all elements on the pre-participation medical evaluation form and attesting that in their opinion the student is physically fit to participate in interscholastic athletics.

No student shall be required to submit to a physical exam if his/her parent(s) or legal guardian shall file with the principal a signed, written statement (affirmed under the penalties of perjury) declining such physical examination on grounds of sincerely held beliefs or practices.  It is required that no student be permitted to participate in practice sessions or in athletic contests until there is on file a parental consent certificate signed by a parent or legal guardian stating that the student has the consent of his/her parent(s) or legal guardian to participate.

7:45AM @ Ballad Outpatient Therapy Center

 

The FREE Athletic Physical for Dobyns-Bennett High School Athletes (rising 9th -12 grades) is set for Saturday May 10th at 7:45am @ Ballad Outpatient Therapy Center (Sullivan Center) on Stone Drive (Old King Giant Plaza). Make sure that you tell your athletes to be there at 7:45am. This mass physical will be the only free physical that will be held this school year for Dobyns-Bennett athletes. Each athlete/player needs to pick up a 2025-2026 High School Medical Authorization Packet with Physical Form from the athletic/activities office at D-B, the athletic training room (located in the Field House) or print out the 2025-2026 High School Medical Authorization Packet with Physical Form. The 2025-2026 High School Medical Authorization Packet with Physical Form is located at the top of this page. Each athlete/player’s Medical Authorization page, KCS Acknowledgement Form, and Physical Medical History portion of the physical form must be filled out and signed by the parent/guardian BEFORE the athlete/player can go through the physical.
