On Saturday, May 15th, 2021 at the Ballad Sullivan Center - Outpatient Rehabilitation & Physical Therapy on Stone Drive. Ballad Health will be offering free athletic physicals for all prospective D-B athletes, including rising 9th graders for the 2021-2022 school year.

 

Ballad Health is requiring Pre-Registration for the physicals. Complete this sign up form to register for the physicals. Please complete this form by May 7th and bring your completed KCS Medical Authorization packet with you on the morning of the physicals. Prior to the morning of the physicals, you will receive your arrival/appointment time via email. Please arrive at your arrival/appointment time on time with your KCS Medical Authorization packet signed and completed.

 

KCS Medical Authorization packets may be printed out from the KCS Athletics website under athletic forms or picked up from the Athletic Office at Dobyns-Bennett High School. Physicals will be conducted on May 15, 2021 from 7:45AM - 10:00AM at the following address:
